A T-score of −1.0 or above is normal. Between −1.0 and −2.5 indicates osteopenia (low bone mass). A T-score below −2.5 meets the clinical definition of osteoporosis.

The Z-score compares your bone density to others of your age, sex, and sometimes body size. A Z-score below −2.0 is considered below the expected range for your age group and may prompt your physician to investigate secondary causes of bone loss — conditions such as hyperparathyroidism, vitamin D deficiency, celiac disease, or long-term corticosteroid use.

Who Should Get Bone Density Testing?

Current clinical guidelines recommend DEXA bone density screening for all women aged 65 and older, and for younger postmenopausal women with risk factors including a family history of osteoporosis, smoking, low body weight, or prolonged corticosteroid use. Men over 70, or younger men with equivalent risk factors, should also consider baseline testing.

However, these are minimum recommendations for clinical screening — not optimal monitoring. Athletes, particularly those in weight-class or aesthetic sports with a history of caloric restriction, young women with menstrual irregularities, and anyone with a personal history of fractures may benefit from earlier and more frequent monitoring.

Proven Strategies to Protect Bone Density

The most effective lifestyle intervention for bone health is resistance exercise — specifically, high-impact and weight-bearing activities that stress the skeleton and stimulate osteoblast activity. Walking, running, tennis, and weightlifting all confer significant bone-protective benefits. Swimming and cycling, while excellent for cardiovascular health, are non-weight-bearing and provide minimal bone stimulus.

Adequate calcium and vitamin D intake are foundational. Most adults require 1,000–1,200 mg of calcium daily and 1,500–2,000 IU of vitamin D3 per day, though individual needs vary. Avoiding smoking, limiting alcohol to moderate consumption, and managing corticosteroid use with your physician are additional modifiable risk factors.

When Medication Becomes Appropriate

For individuals with established osteoporosis or osteopenia combined with high fracture risk, lifestyle modifications alone are often insufficient. Several classes of medication have been shown in randomized controlled trials to significantly reduce fracture risk: bisphosphonates (alendronate, risedronate), denosumab, romosozumab, and anabolic agents like teriparatide.

The decision to begin pharmacological therapy is made jointly between you and your physician, taking into account your T-scores, FRAX fracture risk assessment, medical history, and preferences. Serial DEXA scans are the primary tool for monitoring whether treatment is working — typically performed every 1 to 2 years on therapy.

Monitoring Your Bone Health Over Time

A single DEXA scan gives you a snapshot; serial scans give you the trajectory. The difference between a person losing bone at 1% per year versus 0.1% per year is enormously consequential over a decade, but invisible without regular measurement.

If your T-scores are normal and you have no significant risk factors, the standard recommendation is to retest every 2 years starting at age 65 for women. If you have risk factors, osteopenia, or are on treatment, annual monitoring provides the feedback loop needed to adjust interventions promptly. Don't wait for a fracture to find out where your bone health stands.
